We are delighted to present to you the offerings for "Lifelong Faith Formation."
To register Preschool, Elementary and Junior High Students: Click on "Registration Forms" to the left. Read pages 1 & 2. Print pages 3a-f, fill out and return by mail with payment to Faith Formation Office,1275 B Street, Davis, CA 95616 or in person (office opened 8:30-4:30 pm). Print other pages as needed. To register for Child Care Co-op: Read page 8, print Form 7 and return with payment. High School Confirmation: Registration forms available mid-September.
